Southwestern College Student-Athlete:

Welcome to another exciting year of Builder athletics!  Below you will find all the information needed so that you can start the 2025-26 school year as a Moundbuilder Student-Athlete.  I encourage you to fill these forms out with the help of your parent(s) or guardian.  You are encouraged to get your physical by your family doctor (MD/DO/PA-C/& ARNP ONLY) before coming to campus, as we will not offer physicals on campus.

SPORTS MEDICINE – TO DO LIST

1) Healthy Roster - This is where all our medical paperwork will be found - please reach out so that we can create an initial account for your son or daughter.

2) Physical – Use SC's form sckans.edu/physical and must be done by one of the following -- MD/DO/PA-C/or ARNP.

3) Insurance - LINK Injuries do occur, and we, the sports medicine staff, attempt to provide our student-athletes with the very best possible care that is within our means. Medical bills are incurred when a student-athlete is treated, whether it is locally, during a road trip or by a medical vendor in his/her own home area. The athletic accident insurance coverage provided by Southwestern College is for an accident (a sudden, specific definable event) sustained while participating in the play or official practice of an intercollegiate sport at Southwestern College. This coverage is designed as secondary coverage meaning it is an "excess type" of coverage. All claims must first be submitted to the athlete's insurance/parent's insurance/or guardian's insurance company for payment. The secondary coverage program will then pay the remaining balance according to our policy. You will be required to carry a primary insurance which meets the following:

a. Must provide primary insurance coverage (not payer of last resort)

b. Must be a US based Insurance company

c. Must cover in Kansas**

i. Kaiser – student will need to go to where Kaiser is taken on their own dollar

d. Must cover athletic type injury (college athletics)

e. Provide $35,000 per athletic injury

Coverage that will not work as primary medical insurance (not an exhaustive list) –

a. Medicaid

i. In State – is the payer of last resort

ii. Out of State – won't cover out of home state (only in case of emergency)

b. Payer of last resort (i.e. Military (Tricare)/Indian Health/etc.

c. Health share program (i.e. Christian Healthcare Ministries) – not insurance, but a sharing program

d. International Insurance – must be covered by a US based Insurance Company/not a reimbursement or travel policy/etc.

e. Travel based Insurance - $3,000 per athletic injury generally speaking ($35,000 per injury is our rule)

f. Golden Rule – only provides $10,000 per athletic ($35,000 per injury is our rule)

All Southwestern College student-athletes are required to have primary insurance coverage to participate in intercollegiate athletic activities associated with Southwestern College. If a student-athlete is unable to find an accepted plan or loses their insurance during the academic year, a non-compliance fee will be charged to the student-athlete's bill.

This additional fee is assessed for two reasons, first, to offset expenses related to Southwestern College's secondary accident insurance plan, which covers uninsured student-athletes who are injured, and second, to ensure consistency and fairness for the student-athletes who are complying with the requirement by holding an accepted primary insurance policy and paying the premiums to be insured. Just a reminder, there is no health benefit to our secondary policy.  Insurance must be on file with Southwestern Colleges' sports medicine staff before you are cleared for participation in intercollegiate athletics, which include practice and/or competition.

Proof of primary insurance may be provided up until the add/drop date. If proof of primary insurance cannot be provided a fee will be added for your specific sport to your student account. Additional steps to authorize financial aid in paying for this charge will be required.

In order for our sports medicine staff to provide the best possible care for our student-athletes, it is vital that you provide complete and accurate insurance information. If your insurance information changes for any reason, you will need to update the information as soon as possible with the sports medicine staff.

All non-athletically related injuries and illnesses will be submitted to the athlete's primary insurance, and the remaining balance will be the responsibility of the student athlete and/or parent/guardian. Charges for medical abnormalities, or pre-existing conditions, are the responsibility of the student athlete and/or their parents/ guardians.
